Required Forms by Grade:
If your child is entering:
- Preschool/or new student in any grade—child health examination
- Kindergarten—child health examination, dental examination, eye examination
- 2nd grade—dental examination
- 6th grade—child health examination, dental examination
- ANY student new to Illinois—child health examination, eye examination
Please submit all documents and forms directly to the school. These are due by August 15th.
Forms received after August 15th will be subject to a $50 administrative fee.
