The following health forms are required for the indicated students:

First-time Illinois students from out of state/country: Certificate of Child Health Examination
First-time Illinois enrollees in kindergarten or above: Eye Examination
Preschool: Certificate of Child Health Examination
Kindergarten: Certificate of Child Health Examination, Dental Examination, and Eye Examination
Second Grade: Dental Examination
Sixth Grade: Certificate of Child Health Examination and Dental Examination
If applicable: Illinois Certificate of Religious Exemption
